- A completed North Dakota Application for Concealed Weapons License (Form SFN-9756) or fill the online form and print it.
- Certified proof of the passing of a firearm safety test from an approved test administrator (must pass the test within 30 days of application submission).
- Two passport-style color photographs of the applicant.
- Two original fingerprint cards (obtained from your county sheriff’s office).
- A copy of your active North Dakota driver’s license or identification card.
- Money order or cashier’s check for all associated fees ($60 application fee).
